There is a one-time non-refundable membership fee of $200 to join the practice.

Exceptions
Membership fees are waived for children <30 days old if the mom is a current member. The child must have his or her own full initial intake.

For young girls that have started menstruating, and are less that 18 years of age (and children of clients), there is a 50% discount on the membership fee. Each child must have her own full initial intake.

Do You Accept Insurance, Medicaid, or Medicare? We do not bill insurance, Medicaid or Medicare, but some services may be reimbursable. Working with clients on an in-depth level is not reimbursed by insurance companies, unfortunately. However, I can provide a superbill for you to submit to your insurance company for attempted reimbursement. It is up to each patient to determine if their plan covers consultations, supplements, etc.
Are You My Primary Care Provider?​ No, while I do offer many primary care services, I encourage patients to work concurrently with a primary care provide. I do not provide emergency services, urgent care services or after hours triage. In an emergent situation, I recommend visiting an Urgent Care clinic or Emergency Room as appropriate. We can have a visit afterwards and do any "clean up" afterwards (i.e. probiotics after a round of antibiotics).
Appointment Cancellation/Rescheduling There is a 14-day Cancellation policy for initial appointments, and a 72-hour Cancellation and Rescheduling policy for all follow-up appointments. We reserve the right to charge your credit card the full amount of the visit if it is not canceled or rescheduled by this deadline. All cancellations must be made through your account on the Client Portal. ​
What is Functional Nutrition? Functional Nutrition is about finding the right way for each of us as individuals to eat – using food to maximize the potential for health and reverse dysfunction or disease. There is no single ‘right diet’ that applies to all of us. We have different genetic backgrounds, different preferences, and different lives. We all want to be healthy, but most of us haven’t figured our just how to make food and dietary patterns serve that goal. Functional Nutrition offers the concepts, strategies, and tools to make that happen!
What is Functional Medicine? "Functional Medicine determines how and why illness occurs and restores health by addressing the root causes of disease for each individual."
Do you sell supplements? I use Fullscript to recommend high quality supplements at a 30% reduced rate ot make them more affortable. You are more than welcome to get the recommended supplemnts on your own, but I do shy away from Amazon, as there is no guarantee that the product is in fact from the company on the label.
Can I use my HSA or FSA for my monthly membership or ancillary services? Yes, most insurances will allow this pre-allocated amount to be used for either membership, labs, in-house medical procedures
What is a Nurse Practitioner? A Nurse Practitioner is a registered nurse with a significant number of years of patient care that has chosen to advance his/her education and training by obtaining an extensive medical graduate degree. After sitting for national board certification and qualifying for state licensure, he/she is then considered a primary care provider, much like a physician. Nurse Practitioners are required to declare specialties such as Family Practice, Pediatrics, Acute care, etc. A Family Practice Nurse Practitioner can see patients of all ages, manage preventative, acute and chronic conditions and prescribe medications. In the state of Colorado, Nurse Practitioners are legally able to either work within a practice of many providers or they are able to establish an independent practice.
Do you prescribe medications? Yes, in the state of Colorado & Alaska a Nurse Practitioner is legally able to prescribe all medications of a Family Practice Physician.
Labs & Functional Testing Some of the laboratory services recommended MAY be covered by insurance. I encourage you to talk with your insurer about coverage if you have any questions about what will be covered.
